As the primary source of transformation funding, it’s impossible to ignore sustainability transformation plans (STPs) – however they are not the only game in town. Many of the new care models the Five Year Forward View and Health and Social Care Act 2012 want to see introduced in health and social care, have been around for a while and stem from long-standing collaborations and partnerships.
Tower Hamlets was given ‘Vanguard’ status by NHS England for its multi-speciality community provider programme. In April 2016, Tower Hamlets CCG announced that a new partnership Tower Hamlets Together, would be the new provider of community health services in the borough enabling coordinated care for patients, that sits outside of traditional hospital environments.
In Sutton, the CCG was awarded Vanguard status in March 2015 – one of only six enhanced health in care home vanguards in England. It aims to improve the health and quality of life for all care home residents in Sutton by delivering a new care model that offers older people living in care homes improved and integrated health care and rehabilitation services.
